Online Therapy for ADHD and Neurodivergent Adults in the UK
Online therapy has exploded — but generic providers weren't built for ADHD or autistic brains. Here's what neurodivergent-affirming online support actually looks like in the UK.
Search demand for "online therapy" in the UK has grown to over 5,400 searches a month and is trending upward year on year. NHS Talking Therapies received 1.82 million referrals in 2023/24 — an all-time high — and BACP's 2026 survey found nearly a third of UK 16–24 year olds have now seen a counsellor or therapist.
For neurodivergent adults — ADHD, autistic, dyslexic, AuDHD — that growth is a double-edged sword. Access has improved. But most of the online therapy market was built for a neurotypical model of what "help" looks like. Long silent sessions, cognitive homework, "sit with the feeling" — the standard playbook can miss what an ADHD or autistic nervous system actually needs.
This is a plain-language guide to what neurodivergent-affirming online therapy and coaching looks like in the UK, and how to tell whether a provider is a good fit for you.
Why "online therapy" isn't one thing
The phrase gets used for very different things:
- NHS Talking Therapies (IAPT). Free, evidence-based (usually CBT), waiting lists vary by region. Sessions are often short-course and structured.
- Private online counselling. Registered counsellors delivering weekly sessions over video, typically £50–£90 per hour.
- Therapy platforms (BetterHelp, Kooth, others). Subscription models, variable therapist matching.
- Coaching. Not the same as therapy — coaches work on goals, systems and skills rather than diagnosing or treating mental-health conditions.
All four can be delivered online. All four vary hugely in whether they're actually equipped to work with neurodivergent adults.
What "neurodivergent-affirming" actually means
The phrase gets used loosely. In practice, it means a practitioner who:
- Treats ADHD and autism as differences, not deficits. The goal isn't to make you behave more neurotypically. It's to help you understand and work with the nervous system you have.
- Adapts the format. Shorter, more structured sessions. Written follow-ups. Visual tools. Movement or fidgeting allowed. Cameras optional.
- Understands nervous-system regulation, not just cognition. ADHD and autistic brains regulate emotion through the body first. A therapist who only works with thoughts will miss most of what's happening.
- Names their training. Look for post-qualification training in ADHD, autism, PDA, sensory processing or trauma — not just a general counselling qualification.
Common questions people ask about online therapy in the UK
Is online therapy effective?
For most people, yes. Multiple UK and international studies show video-delivered therapy produces outcomes comparable to in-person for anxiety, depression and many other presentations — especially when the therapist is well-matched to the client.
Is online therapy safe?
When you work with a therapist registered with a UK professional body (BACP, UKCP, NCPS, BABCP) or a coach with a recognised credential and supervision structure, yes. Check the register before you book.
How much does online therapy cost in the UK?
NHS Talking Therapies is free. Private online counselling typically runs £50–£90 per session. Specialist neurodivergent coaching sits in a similar range, sometimes higher for highly specialised practitioners.
Can I get free online therapy?
Yes. NHS Talking Therapies (self-referral in most areas), Kooth (under-25s), Togetherall, and university counselling services are all free at the point of use. Waiting times vary.
How to choose an online therapist or coach as a neurodivergent adult
A short checklist:
- Ask about neurodivergent training on the first call. If they can't name specific training, keep looking.
- Ask what an adapted session looks like — cameras, breaks, written summaries.
- Notice how they talk about ADHD or autism. Deficit language ("suffers from," "afflicted with") is a red flag. Difference-based language is a green one.
- Trust your nervous system. If the first session leaves you dysregulated in a way that doesn't settle over the next couple of days, that's data.
